The Crystal City Independent School District is requesting proposal for its Student Insurance K-12 for the 2022-2023 scholastic year for students in athletics and other UIL activities.

REQUEST FOR PROPOSALS
STUDENT INSURANCE
The Crystal City Independent School District is requesting proposal for its Student Insurance K-12 for the 2022-2023 scholastic year for students in athletics and other UIL activities.
Submit proposals for two types of plans:
(1) Schedule Plans and (2)100% Usual and Customary Plans.

Instructions:
Sealed proposals should be mailed: Attn: Florencio Zepeda, Finance Office, Crystal City ISD, 805 E Crockett St, Crystal City, Texas, 78839 or hand delivered at the Crystal City ISD Administration Building, 613 West Zavala Street, Crystal City by 12:00p.m. Monday, August 1, 2022. Sealed Proposals should be clearly marked outside the envelope: “Student Insurance RFP# 23-30”
Proposals received on time shall be opened and reviwed by the Chief Finance Officer and submitted to the Board of Trustees for consideration as soon as possible after the closing date and time. The Board of Trustees and the CCISD assumes no responsibility for proposals mailed or misdirected in delivery. Proposals received after the closing date and time will not be accepted and will not be part of the process. Faxed or emailed proposals WILL NOT be accepted.
The Crystal City Independent School District reserves the right to accept or reject any and all proposals and to waive all formalities in the process.
